Case Management Services

Mirror ImageAHP has provided case management services since 2006 to people living with HIV/AIDS (PLWHA) in Central and Eastern Brooklyn. This component addresses the needs of HIV positive clients and connects them to needed social and medical services. AHP provides a full range of supportive services, including case management, psychosocial support, health education and risk reduction, and treatment adherence education.

The target population includes HIV-positive persons who are uninsured, Medicaid-ineligible, and out of care. The program also targets immigrants, both naturalized citizens and undocumented.  The immigrant groups in the service area are associated with high rates of HIV.
