Gestión de casos

AHP’s Case Management program is designed to coordinate medical and support services for individuals living with chronic conditions and their families. Our case managers connect participants to primary care providers,  treatment resources, housing, mental health services, psychiatric care, and legal services.

In addition, We provide supportive counseling and low-threshold case management services to address the diverse needs of individuals living with chronic conditions. Our case managers connect clients to essential social and medical services when needed.

The program serves individuals who are uninsured, Medicaid-ineligible, or out of care. We also target immigrants, including both naturalized citizens and undocumented individuals.
