Case Management Services
AHP’s Case Management program is designed to coordinate medical and support services by connecting participants to primary care, HIV treatment including PEP/PrEP resources, housing, mental health services, psychiatric care and legal services. The program provides supportive counseling and low-threshold Case Management services to PLWHA (People Living with HIV/AIDS) and their families.
Case Managers at AHP address the many needs that HIV positive clients bring to the table, and when needed, they connect clients to vital social and medical services.
The target population includes persons who are uninsured, Medicaid-ineligible, and out of care individuals. The program also targets immigrants, both naturalized citizens and undocumented.
