The Case Manager provides in-the-moment, client-centered support and coaching to identify and resolve problems in client’s everyday lives. He offers supportive services to clients seeking harm reduction services, including an array of referrals to services such as detox and drug rehabilitation programs. He addresses clients’ emergency housing needs by helping with short-term housing referrals. He educates clients about the in-house buprenorphine program as well as methadone maintenance/methadone to abstinence programs to determine which program best suits the client’s preferences and needs, and then refers them to their requested program. He trains individuals at risk on how to safely use Narcan and supplies them with an inter-nasal naloxone kit.
