The Patient Navigator provides support to the Supportive Counseling and Family Stabilization Program. The Patient Navigator works directly with Case Managers to ensure client needs are met. She screens for mental health and substance use related issues. She provides support to the agency’s individualized and family-focused supportive counseling to facilitate continued engagement in medical care and treatment adherence. The Patient Navigator performs education sessions with clients about the benefits of engaging in mental health and substance misuse treatment. She maintains and develops relationships with medical partners and medical professionals that serve our clients and participates in case conferencing to ensure optimal outcomes. The Patient Navigator works with Case Managers to ensure the maintenance of records including accurate and clear documentation of all outreach, education, client activities, and navigation activities.
